Passwords and expiry from the share dialog

Set or remove a document's password and expiry from the share dialog, including a new one-hour expiry.
The share dialog's More options now sets a password and an expiry for Markdown documents as well as files, including On a date…, and the new one-hour option. The dialog also shows the document's real address.
For scripts, PATCH /api/v1/documents/{slug}/settings changes the password and expiry without resending the content.
A document published without a title now takes one from its first heading or first line.
